We’re delighted to sponsor today’s Link Worker Conference & first national Social Prescribing Link Worker Day
Today’s event brings together professionals delivering a social prescribing Link Worker role to promote best practice, and provide an arena for sharing experiences and learning from around the country.
Connecting Link Workers to Communities
We want to ensure that Link Workers are connected with local community organisations, and can easily refer their clients into quality assured community services. We’re here to share our experience of working in the VCSE sector, and the different social prescribing delivery models, as well as connect Link Workers with our Simply Connect Community Partners to access our network of 100,000 VCSE organisations around the country.

“We’re delighted to be supporting this exciting new campaign to celebrate the important role of Link Workers, as part of a whole systems approach to personalised care. Link Workers, and staff in wider community agencies who do this as part of their role, spend time with individuals to really understand their needs and support them into appropriate community services.
Whilst the formal ‘Link Worker’ role is new to primary care – thanks to new funding from NHS England for every Primary Care Network in the country – many staff within the voluntary sector have been delivering social prescribing in communities for many years. This includes staff working in Volunteer Centres, in mental health support agencies, domestic abuse support groups, larger organisations like Age UK, to the smaller grass roots community groups – all working to empower people to take control of their own health and wellbeing.
Link Workers really need to understand their local community – which community services are available on their doorstep, how to access them, and support clients into these services. They also need to have the right tools to track and manage their clients, which is where we come in.”

The Link Worker Role
Working across statutory and voluntary sectors, the ‘Link Worker’ role requires a unique set of skills and knowledge – providing support to individual’s to understand and address their personalised health needs, as well as having an understanding of their local VCSE sector, being able to work well across sectors, identify gaps in service provision, and liaise with primary care and multi-disciplinary teams.
