Simply Connect chosen for new Interreg Social Prescribing project

We’re delighted to be Digital Partner for Connected Communities – an Interreg Social Prescribing project.

 

Connected Communities involves nine partners from the UK and France to enable older people to better manage their own health and social care, and prevent or delay the need for formal health and wider care services.

 

The project proposes to boost older people’s ability to help themselves, increasing happiness, decreasing loneliness and reducing the demand for social services.

 

Our Social Prescribing platform will be used by partners to capture data and collate insights to evidence the impact of social prescribing. We’ll be working closely with the University of Essex, who are evaluating this project. This project builds on our existing work with Medway partners – Medway Council and Medway Voluntary Action.
