Simply Connect supports new ‘Better Connected’ service in Medway

Link Workers from Better Connected – a new social prescribing service in Medway – are now trained and ready for their launch next week!

More Link Workers, and Community Navigators from the VCSE sector, will be trained in the coming months.
The teams – part of A Better Medway from Medway Public Health – will use our Simply Connect social prescribing platform to support their work with clients to reconnect them with their community, develop new friendships and take control of their health.

The service launches on Monday 16th March and will specifically work with people aged 65+ years who are socially isolated.
The team will receive referrals and self-referrals, and are also running workshops for health professionals from the social care, community and voluntary sector, to ensure as many people as possible are able to benefit from the service.
