Simply Connect brings the community to you, facilitating strategic partnerships with the voluntary and community sector
The voluntary sector is at the heart of social prescribing. Our established model combines local delivery partners and data, an understanding of local health needs and our innovative social prescribing platform to deliver a local solution that addresses health inequalities, tackles social isolation and improves health and wellbeing.
Together, we can reduce pressure on health and social care services by connecting local people with quality assured local community services and support.
Our Simply Connect Social Prescribing model aims to:
+ Help people live more healthier, active and happier lives by supporting into community services
+ Be inclusive – helping to make social prescribing accessible for hard to reach communities, working with our voluntary sector network
+ Reduce dependency on NHS services, GP visits, A&E visits and demand on social care services
+ Take an Asset Based Community Development approach (ABCD), building on what’s already strong locally – the people, community services and assets
+ Improve health and wellbeing outcomes for local people and communities
+ Enable people to be proactive in managing their own health conditions and wellbeing.
