Did you know our ‘Connect Well’ social prescribing platform has been supporting self-referrals since 2017?
For over a year now, residents in Essex have been using our ‘Connect Well’ platform to access local support services to help improve their health and wellbeing.
By accessing the application on their smart-phone or tablet, people can quickly make a self-referral directly into quality assured local community-based care and services, making it really easy for them to get involved in social activities, sports clubs, support groups and local volunteering opportunities.
Encouraging this ‘self-care’ approach is so important for empowering people to be able to take control of their own health and wellbeing, and is at the heart of a green paper announced today by Matt Hancock, Health & Social Care Secretary entitled “Prevention is better than cure”.
Focused upon prevention that aims to stop people from slipping into poor health, Matt Hancock says,”Prevention is also about ensuring that people take greater responsibility for managing their own health”.
